International Health Certificates

If you plan to travel with your pet to another country, you must schedule a regular exam with our doctor at least one month before traveling to ensure that all of the necessary vaccines and paperwork are available to us. At this time, you must bring all vaccination history (if vaccines were done at a clinic other than 4th Street Pet Hospital), and the requirements for flying to the country you will be taking the pet to.

Please note: We may not have all the information required for travel for every state or country as the rules for these states or countries are subject to change. We are only able to fill out the international health certificate with the procedures or vaccinations done at 4th Street Pet Hospital, or with provided documentation with other clinics.

Pricing
Travel consultation ~ $89.00

This is a consultation to go over where you are traveling, and go over all information and treatments that will be needed to travel.

International Health certificate ~ $450.00

Additional Fees that may apply:

USDA online submission by a USDA accredited veterinarian ~ $250.00
Rush fee if traveling under 21 days ~ $150.00

**Once the certificate is submitted to the USDA through the online portal we have no control on when it actually gets approved with the USDA. It is the owners responsibilities to make sure this process is started in a timely manner to get all correct information to the USDA for the smoothies transition.
